Description
of District
Cleveland County is typical of many rural North Carolina counties
with a widely dispersed population of nearly 86,000 residents spread
out over 466 square miles. Census data for 1990 showed a population
of less than 15,000 for the county seat of Shelby where the majority
of human services agencies are found. Located in the Southwestern
corner of the state, Cleveland County still has over 500 farms,
yet employs 14,500 workers in over 250 manufacturing firms. Nearly
one third of these workers rotate shifts or work second or third
shifts. Cleveland County Schools, one of the three school systems
in the county, serves 9000 students who live in the rural areas
and small towns The Cleveland County School system operates 11 modern
facilities including seven K-5 schools, two middle schools (grades
6 through 8), and two high schools.
Mission
Cleveland County Schools believes that all students can learn and
that it is our responsibility to teach learning for all. The mission
of Cleveland County Schools, in cooperation with the community,
is to educate all students to achieve academic and social success
in a caring environment.
The Cleveland County School system is recognized as being a state
award winning Tech Prep/Job Ready Programs, as an innovator in developing
successful school based health centers in securing a 5 year federal
grant to expand a North Carolina System of Care initiative to unify
and marshal local services and resources to promote the success
of children with emotional disturbances and to support families.
Description of Project
Cleveland Regional Medical Center led a planning process to establish
a school based and school linked Health Center at Crest Middle School
in the Cleveland County Schools which will provide integrated health
services for adolescents. This strategic planning and implementation
process involves Cleveland Regional Medical Center, the Cleveland
County Health Department, Cleveland Center (Area Program for Mental
Health, Developmental Disabilities and Substance Abuse Services)
and Cleveland County Schools as primary providers of services to
this student population. Currently, on campus site preparation and
hiring of personnel are nearing completion. The staffing will be
provided from Cleveland Regional Medical Center, Cleveland County
Health Department, Cleveland Center (area Mental Health), Cleveland
County Schools, and CLECO Primary Care network. Intensive staffing
of this school based health center will provide an enhanced level
of service delivery and capacity to seek both Medicaid and private
provider medical reimbursement for billable services.
Three Year Goal
Provide comprehensive health services to students at Crest Middle
School in Cleveland County. The emphasis will be on primary health
care, health promotion, mental health services, nutrition counseling
and the delivery of appropriate health education interventions that
will positively impact on healthy lifestyle behaviors of students.
Outcome Objectives

Reduce
the percentages of pregnancies, including repeat pregnancies,
among the students using SBHC (school based health centers)
services by 50% within three years. |
| 2. |
Reduce
the rate of absenteeism by 2% per year for three year totaling
6% at the conclusion of the three-year grant period. |
| 3. |
Reduce
the frequency/severity of substance abuse use by 10% of the
students participating in the SBHC by year three. |
First Year Outcome Objectives
Process Objectives:
| 1. |
Implement
primary health care services enrolled at one middle school for
the three-year grant period. Provide primary care to a minimum
of 400 students in the SBHC by December 31, 1997. |
| 2. |
Implement
mental health services to students enrolled at one middle school
for the three-year grant period. Provide assessment and treatment
where indicated for substance abuse and other mental health
issues to 20% of the population served by the SBHC by December
31, 1997. |

Implement
an outreach prevention plan for students identified by school
counselor's data as being "at risk" for school dropout, enrolling
at least 50% of those identified students by June 30, 1998. |

Improve
the current SBHCs and the Health Department with one additional
middle school by December 31, 1997. As appropriate, participate
in the development and implementation of a country-wide CHN
(Computer Health Network). |
Outcome
Objectives:
| 1. |
Provide
nutritional assessment and counseling to 20% of the students
participating in the SBHC by June 30, 1998. |
| 2. |
Decrease
by 10% the "Primary Care emergency room visits by students participating
in the SBHC by June 30, 1998 and increase active student participation
in the CLECO Primary Care Network by 10% by June 30, 1998. |
