Medical Coverage
Includes a choice of Point-of-Service (POS), Preferred Provider Organization
(PPO) and Health Maintenance Organization (HMO)/Exclusive Provider Organization
(EPO) plans, depending on where you live. Coverage includes prescription drug
and mental health benefits.
- POS plans provide medical treatment and services through doctors, hospitals and other providers working together as a network to provide quality medical care. In POS plans, you choose a primary care physician from the network to help coordinate all your medical needs. Out-of-network care is covered, but using in-network services maximizes your benefits.
- PPO plan provides medical treatment and services through a national network of doctors, hospitals and other providers working together to provide quality medical care. With a PPO, each time you need care you can choose to receive treatment through the PPO network or through providers outside the network. Using in-network services maximize your benefits.
- HMO/EPO plans provide medical treatment and services through doctors, hospitals and other providers working together as a network to provide quality medical care. With most HMOs and EPOs, you choose a primary care physician from the network to help coordinate all your medical needs. Generally, an HMO or EPO pays no benefits if you receive care without your primary care physician's approval, except in emergency or urgent situations.
Dental Coverage
Offers a dental plan which covers preventive, basic, major and orthodontic
services.
